Liability Release for activities sponsored by Horizon Unitarian Universalist Church

I hereby grant permission for my child/children to participate in activities sponsored by Horizon Unitarian Universalist Church (HUUC). I understand that all reasonable health and safety precautions will be taken, but in the event of an accident, this form will release HUUC, its staff, and its volunteers of all liability claims not covered by current HUUC insurance coverage. In the event that an emergency should occur while my child is participating in activities sponsored by HUUC, I hereby grant my permission to the person in charge of the activity to do whatever is necessary to ensure the safety and wellbeing of my child / children and I hereby authorize said person(s) to seek whatever medical care is necessary. I agree to assume all responsibility for any such medical expenses.

Our Whole Lives (OWL) comprehensive sexuality education

This school year Horizon plans to offer the Kindergarten - 2nd grade level of OWL beginning in January 2024. The yearlong 7-9 grade OWL course is only tentative at this time pending registrations.
Please indicate your interest for your child / children to participate in OWL this school year.

If not offered this year, this level of OWL will be offered next school year beginning in August 2024) Our family is interested in both OWL levels for the 2023-24 school year.

I hereby give permission for my child/children to participate in the Our Whole Lives comprehensive sexuality education courses offered at Horizon Unitarian Universalist Church in Carrollton, Texas during the 2023-24 school year. I understand I must complete this registration form and attend a mandatory parent/guardian orientation session before my child(ren) will be allowed to participate in the course(s). I also acknowledge that attendance is important and my family will strive to send our child(ren) to a minimum of ninety (90) percent of OWL sessions unless other arrangements are made with the OWL leaders.
